CAMP PARTICIPANT TERMS & CONDITIONS, WAIVER & RELEASE FORM

I, the undersigned, have voluntarily enrolled my child (the “Participant”) in a soccer camp, clinic, or program (the “Camp”) provided by Salas Training LLC, a New York-based company, with instruction by Coach Salas and her staff(collectively, the “Trainer”). The Camp may include, but is not limited to, soccer training, games, and physical exercise (the “Services”). By signing this form, I accept the Terms & Conditions set forth below.

In consideration of Trainer providing these Services, I, on behalf of myself and my agents, heirs, and assigns, hereby release, discharge, and hold harmless Trainer, Coach Salas, her staff, and all affiliated agents, contractors, employees, and representatives from any and all claims, demands, damages, actions, or causes of action, whether known or unknown, arising from or related to participation in the Camp, including injuries that may occur.

Waiver & Release:
I acknowledge that the Participant is physically fit and mentally capable of participating in soccer and exercise activities. I understand that soccer and physical exercise involve inherent risks, including but not limited to: falls, collisions, concussions, heat-related illness, abnormal blood pressure changes, fainting, heart attack, stroke, serious injury, or death. I voluntarily assume all risks associated with participation, including risks arising from the condition of the facilities or equipment and the actions of other participants.

I waive, release, and forever discharge Salas Training LLC, Coach Salas, her staff, and their respective officers, agents, contractors, and employees from all claims or liability for damages, injury, or loss, whether caused by negligence or otherwise. I understand that no refunds will be given for injuries sustained during Camp.

Medical Treatment Consent:
I authorize the staff of Salas Training LLC to seek medical attention for the Participant in case of accident, injury, or illness. I agree to be responsible for all costs associated with such medical care.

Photography & Promotional Use:
I understand that the Participant may be photographed or recorded during Camp activities. I grant Salas Training LLC and its staff permission to use such images or videos for promotional purposes, including advertising, social media, and publications. I understand no compensation will be provided for the use of these images, and I relinquish any rights to privacy, publicity, or other claims related to the images. If I do not wish for the Participant to be photographed or recorded, I must notify the Trainer in writing prior to participation.

Cancellation & Refund Policy:
Camp fees (minus any transaction fees) are refundable only if written cancellation is received at least 7 days before the start of the Camp. No refunds will be issued for cancellations made less than 7 days prior, for no-shows, or after the Camp has begun. Camps may be canceled or rescheduled due to insufficient enrollment or unforeseen circumstances. In such cases, Salas Training LLC’s liability is limited to a refund of the Camp fee minus transaction fees.

Weather Policy:
Camps run rain or shine unless severe weather (e.g., thunderstorms) occurs. No refunds are issued for rain. In the event of dangerous weather during Camp, participants will be asked to leave the facility with their parent or guardian.

Governing Law:
This agreement shall be governed by the laws of the State of New York, without regard to conflicts of law. Any disputes arising under this agreement shall be resolved in the courts of New York County or the applicable federal courts within New York State. The prevailing party shall be entitled to recover reasonable attorneys’ fees and costs.

Entire Agreement:
This document constitutes the entire agreement between the parties and supersedes all prior communications or agreements regarding the Camp. If any provision is unenforceable, the remaining provisions shall remain in full effect.

I acknowledge that I have thoroughly read this form and fully understand that it is a release of liability. By signing, I waive any right to bring legal action against Salas Training LLC, Coach Salas, or her staff for negligence or claims related to participation in the Camp.

This is an important legal document explaining the risks assumed by participating in soccer, sports, and/or exercise activities. If any part of this form is unclear, it is my responsibility to seek clarification before signing.
