TRAINING FORM

PARTICIPANT AGREEMENT, WAIVER, AND RELEASE FORM

I, the undersigned participant, voluntarily enroll in a soccer camp provided by one or more coaches from Salas Training. LLC, a New York State corporation (referred to as the "Trainer"). This enrollment may include various activities such as soccer training, exercises, and related services (collectively referred to as the "Services"). In consideration of the Trainer's agreement to provide instruction and training, I hereby release and discharge Trainer, along with its agents, heirs, assigns, vendors, contractors, and employees, from any and all present or future claims, demands, damages, rights of action, or causes of action arising out of or connected with my participation in these activities, including any resulting injuries.

THIS WAIVER AND RELEASE OF LIABILITY INCLUDES, WITHOUT LIMITATION, INJURIES WHICH MAY OCCUR AS A RESULT OF (1) EQUIPMENT BELONGING TO TRAINER OR TO MYSELF THAT MAY MALFUNCTION OR BREAK; (2) ANY SLIP, FALL, DROPPING OF EQUIPMENT; (3) AND/OR NEGLIGENT INSTRUCTION OR SUPERVISION.

 

I acknowledge that sports and exercise activities, including soccer, are inherently hazardous, involving risks of injury, abnormal changes in blood pressure, fainting, and a remote risk of serious disability, heart attack, stroke, or death. Despite these inherent risks, I voluntarily choose to participate with full knowledge, understanding, and appreciation of the associated dangers, assuming all risks, regardless of severity, or potential fatality.

I am aware of the recommendation for a pre-activity medical examination, and I acknowledge that I am participating in these activities at my own risk, even if I have chosen not to obtain a physician's consent. I expressly assume and accept all risks associated with participating in sports, fitness, training, and exercise activities with Trainer and agree to waive any protection provided by applicable statutes that may limit the scope of a general release.

Furthermore, I understand that during these activities, I may be photographed or filmed. I grant permission for the use of my name, image, and/or likeness without expecting any compensation. I release Trainer and affiliated parties from any claims arising from the use of photographs and/or videos, including claims for libel or invasion of privacy. If I prefer not to be photographed or filmed, I agree to inform Trainer in writing before the activity.

I agree to pay Trainer the full cost associated with participating in the Services provided.

 

I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, the coaches, trainers, volunteers, sponsors and advertisers, from any and all liability incurred in the conduct of, and participation in, their soccer programs. This includes owners, lessors, and lessees of premises, municipalities, government agencies, successors, heirs, and assigns. I have completely read this document and fully understand its contents. I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily. My signature attests to this on behalf of myself and my executors, personal representatives, administrators, heirs, next-of-kin, successors, and assigns.

As the parent and natural guardian or legal guardian of the participant, I hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the participant (player/minor) named above. I hereby bind myself, the minor, and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for, and on behalf of, the minor in the execution of this Waiver of Liability and Release.
